Sending Cases

We pay for UPS pick up and delivery for in zone dentists to a maximum of $10 per unit. For out of zone and out of state dentists, the policy is the same. If shipping costs exceed $10 per unit for any reason, the doctor will be billed the difference.

Shipping Instructions

1. Call for shipping labels (818) 758-3557 x 102

2. Place the label on the top of the box. Make sure that the barcode is placed on a flat surface.

3. Pick up:

   1. Give to a UPS Driver
   2. Drop in a UPS drop box
   3. If "a" or "b" do not apply, call us so we can arrange for a driver to pick up your case(s). (Need one bussines day notice for "c")

Note: UPS Label is one time use only. Every effort will be made to get cases done in time to use the most economical shipping possible. Scheduling demands both internal and external may require more costly shipping fee.
